We provide personalized guidance based on your needs and goals. Goldfields Community Care works with you to recommend services through your Home Care Package, coordinated with Trilogy Care, a registered Support at Home provider.
How do I arrange an aged care assessment?
Check your eligibility through My Aged Care online or by calling 1800 200 422. If eligible, an assessor will visit your home to determine the level of support you need.
How do I choose Goldfields Community Care as my care coordinator?
After approval, My Aged Care sends an assignment letter with a referral code. Provide this code to Goldfields Community Care, and we will coordinate your services through Trilogy Care. You have 56 days from the assignment letter date to select your provider and sign your Home Care Agreement.
Is Goldfields Community Care government approved?
Goldfields Community Care is not a registered provider. We partner with Trilogy Care, a registered Support at Home provider, ensuring all government standards for aged care are met.
Can I choose my care team?
Yes. We match your care team to your preferences and needs. Your team may include support workers and nursing staff, prioritising consistency, continuity, and respectful relationships.
How soon can services start?
Once funding and referral are confirmed, we aim to have services in place within days, ensuring a smooth and prompt start to your care.
What services can Goldfields Community Care help me access?
We coordinate a range of in-home supports through Trilogy Care, including personal care, household tasks, meals, social support, and allied health services. All tailored to your goals and Home Care Package.
How often is my care plan reviewed? Your care plan is reviewed regularly, at least quarterly or sooner if your circumstances change. To ensure your supports remain appropriate and effective.
Are there entry or exit fees?
No. Goldfields Community Care does not charge entry or exit fees, ensuring your funding is fully used for the care you need.
What if I want to change my services?
We encourage open communication. Contact Goldfields Community Care, and we will work with Trilogy Care to adjust your services promptly and respectfully.
