Connect With My Aged Care- Step 1: Start by registering with My Aged Care so an assessor can determine your support needs and funding level. If you need guidance, we’re here to help so you understand each step and know what to expect.
Create Your Care Plan - Step 2: Once you’re approved, we work with you to design a personalised care plan that reflects your goals, routines, and preferences. Whether you need personal care, domestic assistance, nursing support, or help staying connected — your plan is tailored to you.
Receive Ongoing Support - Step 3: After services begin, we stay in touch to make sure everything is working well. We coordinate your care, adjust supports as your needs change, and ensure you always feel informed, respected, and supported.
